Bilirubin (also known as unconjugated or indirect bilirubin )is formed when red blood cells die and their hemoglobin is broken down within the macrophages to haem and globins. In diseases where too much hemooglobin is broken down or the removal of bilirubin does not function properly, the accumulating bilirubin in the body causes jaundice. When bilirubin is detected in the urine, bilirubin may signal underlying liver disease that can result in jaundice (excessive bilirubin stains the fatty tissues in the skin, causing a yellow appearance

Indirect Bilirubin either insoluble form, unconjugated bilirubin(indirect bilirubin), or bound to glucuronic acid to form the so called conjugated bilirubin known as (direct bilirubin or bilirubin conjugate). The indirect form is insoluble and indirect bilirubin is transformed into a soluble or direct form, in the liver. Total and direct bilirubin levels can be measured from the blood, but indirect bilirubin is calculated from the total and direct bilirubin.
The terms "direct" and "indirect" reflect the way the two types of bilirubin react to certain dyes. Conjugated bilirubin is water-soluble and reacts directly when dyes are added to the blood specimen. The non-water soluble, free bilirubin does not react to the reagents until alcohol is added to the solution. Therefore, the measurement of this type of bilirubin is indirect. Test results may be listed as "BU" for unconjugated bilirubin and "BC" for conjugated bilirubin. Total bilirubin measures both BU and BC.
Bilirubin urine test is based on the coupling of bilirubin with 2.4-dichlorobenzene diazonium salt in a strong acid medium. Bilirubin test color changes from light tan to pinkish-purple. No bilirubin is detectable in normal urine by even the most sensitive methods
