Direct Bilirubin, Bilirubin Conjugate, direct reacting bilirubin is a highly water soluble, high purity, synthetic diaurine bilirubin derivative. The chemical behavior of bilirubin conjugate direct reacting bilirubin compound with regards to diazotizing reagents and conditions is identical within experimental limits to that of purified natural bilirubin glucuronides. The solubility and chromatographic behaviors of bilirubin conjugate(d) product are comparable to the glucuronides, indicating similar polarities and solvation properites. Clinically the most important characteristic of direct reacting bilirubin to bilirubin conjugate is it diazo reactivity in the absence of accelerators.
Bilirubin is either in the insoluble form, unconjugated bilirubin (also indirect bilirubin), or bound to glucuronic acid to form the so called CONJUGATED BILIRUBIN (also direct bilirubin or bilirubin conjugate). The indirect form (bilirubin)is insoluble and bilirubin is transformed into a soluble or direct form (conjugated bilirubin), in the liver. Total and direct bilirubin levels can be measured from the blood, but indirect bilirubin (unconjugated) is calculated from the total and direct bilirubin. The terms "direct" and "indirect" reflect the way the two types of bilirubin react to certain dyes. Conjugated bilirubin(direct bilirubin) is water-soluble and reacts directly when dyes are added to the blood specimen. The non-water soluble, free bilirubin does not react to the reagents until alcohol is added to the solution. Therefore, the measurement of this type of bilirubin is indirect. Test results may be listed as "BU" for unconjugated bilirubin and "BC" for conjugated bilirubin. Total bilirubin measures both BU(unconjugated bilirubin, indirect) and BC(bilirubin conjugate, direct).

The formula weight of bilirubin (indirect bilirubin) is 584.7, that of the Bilirubin conjugate (direct bilirubin) is 842.9. The ratio of the weight of Bilirubin to that of Bilirubin Conjugate is 584.7/842.9, which is .694. Therefore theoretically, Bilirubin Conjugate (direct bilirubin) should contain 69.4%, by weight of direct reacting Bilirubin.
